Document Create

When I go to create a New Document the application throws an Internal Server 500 error
Reason is:
ActiveRecord::UnknownAttributeError (unknown attribute:
file):
app/controllers/documents_controller.rb:38:in new'
app/controllers/documents_controller.rb:38:increate'
Also I could not figure out from the documentation where Documents are stored, are they stored in the database?

Ubuntu 11.10
Ruby 1.9.2 installed via RVM
Rails 3.1.2
Flagship has its own gemset through RVM
